Rivendell Behavioral Health Services offers, at no charge, confidential, level-of-care assessments to assist in determining the appropriate treatment recommendation. This service is available for children and adolescents who may be experiencing emotional, behavioral, and/or substance abuse problems.

The initial assessment focuses on the individual's present situation, prior treatment, history of the problem, and current fuctioning level.

Mental health professionals from Rivendell facilitate the initial assessment with the client and their family/guardian. Each assessment is then evaluated by the physician to obtain a recommendation for the appropriate level of care. That recommendation is then given to the family with a referral that meets the client's clinical, financial, and geographic needs.

The Assessment & Referral Department coordinates the scheduling of all assessments. Emergency assessments are available 24 hours a day, 7 days a week on site at the hospital. In some non-emergency cases, mobile assessments may be arranged at an alternative site, such as a school, physician's office, or social service agency.

The level of care assessment does not include medical evaluations, psychological testing, drug testing, or court ordered DUI assessment. If these are services that you are seeking, a referral to the appropriate agency will be made.
