Admissions

• Rivendell's Assessment & Referral Department coordinates all requests for care. These staff members arrange for the initial level-of-care assessment to be completed.
• Referrals can be made by physicians, therapists, caseworkers, ministers, teachers, family members, or other concerned persons.
• Referrals can be made 24 hours a day, seven days a week.
• For non-emergency situations, Rivendell's mobile clinicians can facilitate the assessment in the patient's hometown or county.

By providing a level-of-care assessment at no charge, Rivendell encourages persons with concerns to take the initial step and call for help. All assessments are staffed with a physician at Rivendell for appropriate treatment recommendations.

Take Exit #28 from I-65. Turn left at the first traffic light onto Corvette Drive. Follow Corvette Drive for one mile until it ends at Porter Pike Road. The facility is directly across the road. Turn right onto Porter Pike Road and then make an immediate left into the drive.
